Bringing up end-of-life issues with your family can be challenging. Making your wishes known in legal documents can be even tougher. Yet, having the conversation now, while you are well and not in a crisis, will help ensure that your wishes are honored. By attending an education session, you will learn how crucial it is to take action now to specify how you want to be cared for during your final days.

It is also valuable if you are ever a victim of a serious injury or illness even temporarily.
During the first hour, the educational session, legal, health care and faith perspectives will be discussed to help you with planning. During the second part of the event, certified facilitators, NOTARY and witnesses will be available at no charge to assist you with completion of your Health Care Power of Attorney and Living Will documents.

If you  have already completed your Health Care Power of Attorney and/or Living Will, bring them to the Clinic and we will make them a part of your Vidant Electronic Health Record.
