Cardiovascular and Pulmonary Rehabilitation

If you are recovering from a heart attack, heart or lung disease, or recent surgery, the road ahead can look challenging. You may feel anxious about your health and be concerned about how you're going to manage life and get back to doing the things you love.

Cardiovascular and Pulmonary Rehabilitation is designed to get you moving in the right direction. We'll help you understand your condition and what you need to do to get better. You'll learn how to cope with your diagnosis, lower your risk of future problems and return to an active, productive life. In addition to close communication with your doctors, the program also gives you the chance to interact with other patients who share similar experiences.

Our programs focus on the following keys to healthy heart and lungs:

Nutrition — well-balanced diet, weight management, nutrition assessments

Behavior modification — managing risk factors like smoking, high blood pressure, elevated cholesterol, diabetes, excess weight and sedentary lifestyle

Cardiovascular rehab can help patients who have had a heart attack or who suffer from acute coronary syndrome, angina, artherosclerosis, cardiomyopathy, congestive heart failure, coronary artery disease or peripheral vascular disease. It may also be recommended for people who have undergone angioplasty or stents, bypass surgery, percutaneous coronary intervention (PCI), and valve replacement or repair, including transcatheter aortic valve replacement (TAVR) . It helps control symptoms such as chest pain or shortness of breath, and it can stop or reverse damage to blood vessels. It focuses on improving your stamina and strength and getting you back to your usual activities. The goal of cardiovascular rehab is to help you live longer and reduce the chances for another heart attack. Pulmonary rehab is for patients who have lung problems such as asthma, bronchitischronic obstructive pulmonary disease (COPD)emphysema, pulmonary hypertension or sarcoidosis. The goal of pulmonary rehab is to reduce and control breathing difficulties and other symptoms of lung disease and to help participants achieve their highest level of independence and physical function. Individual treatment plans include education, exercise, breathing retraining and the development of coping and support skills. how the program works

Once we receive your doctor's referral, an appointment will be scheduled for your assessment, including a review of your medical history and medications. On your first visit, you will be oriented to the program, staff and equipment.

Our staff includes respiratory therapists, registered nurses, exercise professionals and dietitians. You will also be assigned a nurse case manager who will make sure that your progress through the program is as smooth as possible. Every two weeks the staff will discuss your progress and update your treatment plan, which is reviewed by our medical director.

Throughout your time in the program, we will be preparing you for your transition back into the community. We will make sure that you know what comes next and how to take care of yourself at home. One of the major goals of cardiovascular and pulmonary rehabilitation is planning how you will maintain your activity and other health lifestyle changes once you complete the program.

Come ready to exercise
You will receive an individualized exercise program that takes into consideration any joint, neck or back pain you have, as well as any other limitations such as shortness of breath, chest pain or fatigue. Each program is approved by our medical director. Please wear supportive shoes and comfortable clothing.

Family and support system involvement
We encourage you to invite anyone involved in your health care to attend an education session or to set up a meeting with your case manager to receive more information about your care. We know that your loved ones are important members of your health care team.

Insurance coverage
Most insurance companies will cover most, if not all, of our services. You will receive insurance information at your first session and your questions will be answered.
