How to Read Your Statement

The statement below is the type that provides a detail of your charges and lists the insurance company(ies) that have been billed, based on the insurance information you provided.  Account information is always located in the upper right corner. The name of the hospital where you received services will always appear in the upper left corner. You are not asked to pay the hospital anything until your insurance company has processed your claims.
