Community health programs are designed to make wellness and prevention resources accessible right in your own community. Every member of your family can improve their health by eating well, getting regular checkups and vaccinations, and staying physically active. Healthier habits help your body handle the stress of recovering from an illness or injury. Some habits, like washing your hands regularly, can even reduce the chances of getting sick in the first place.

Community Health Programs

Longstanding initiatives have given us a head start on many community health improvement strategies encouraged by health care reform. Services provided by ECU Health Medical Center’s Community Health Programs include primary, secondary and tertiary prevention initiatives for the pediatric, adult and older adult populations.

Community Health Improvement

Health care reform has elevated the importance of prevention, ensuring that not-for-profit hospitals address community health improvements and associated social determinants of health with population health. Community Health Improvement (CHI) at ECU Health Medical Center is designed to facilitate and coordinate collaborative programming with internal departments and services as well as external community partners to align with the most recent Community Health Needs Assessment (CHNA) priorities.

The NCDHHS, DPH and the NC Institute of Medicine developed the Healthy NC 2030 objectives in order to achieve long-term sustainable improvements in the health and well-being of North Carolinians. The focus has transitioned from individual health topics to health equity and overall drivers of health outcomes. Research has revealed that clinical care accounts for approximately 20% of health outcomes. Focusing on the physical environment and social and economic factors can result in more improved health outcomes. Using the “Healthy NC 2030: A Path Toward Health” as a framework, the ECU Health Medical Center determined health improvement priorities in fiscal year 2022.

These include:

  • Access to Care / Social Determinants of Health (Individuals living below 200% FPL)
  • Healthy Lifestyles (Limited access to healthy food)
  • Mental Health (Adverse Childhood Experiences)

Senior Services

Older Adults are the fastest growing segment of the population, both nationally and locally. In recognition of the need to serve this important population, ECU Health Medical Center partners with community-based agencies to create special programs that identify and address senior health and social support needs, improve access to care, and enhance the quality of life for older adults.

The Senior Services program assists those aged 55 or older lead healthier, more independent lives. Services include:

  • Health and social support education unique to needs of seniors
  • Chronic disease screening and referral
  • Dementia Friendly Initiative
  • Alzheimer’s support group facilitation
  • Monthly GoldPath Senior’s Program (Third Thursday of each month at 10 a.m.)
  • Medicare counseling and Part D enrollment
  • Resources for caregivers
  • Advanced care planning support
  • Fire and fall prevention awareness
  • Monthly GoldPath newsletter

School Health Program

Children and adolescents can experience a wide range of issues – physical, social, emotional and behavioral – impacting their ability to participate and be successful in school. School Health Program nurses employed by ECU Health Medical Center work with pre-K through 12th grade students in Pitt County Schools to help students, parents and teachers manage these challenges and improve student health and academic performance. They do this by coordinating programs that promote good health among students and team members as well as providing case management services to students with chronic and complex medical needs.

Activities include:

  • Assessment of student and team members health needs
  • Assistance with access to medical, vision and dental care
  • Coordination of care for students with chronic health issues, including development of health plans and emergency action plans
  • Collaboration between the school and health care providers
  • Assistance with transitioning students back to school following serious illness or injury
  • Management of the medication administration process
  • Participation in 504 and Individualized Education Programs for health issues
  • Provision of health-related counseling and education
  • Coordination of educational programs on disease and injury prevention
